Tag Line 1; Your first and last name.
Tag Line 2; Your home street address.
Tag Line 3; Your home city, state, zip.
Tag Line 4; Emergency contact phone other than your home phone.
Tag Line 5; Any medical allergies, conditions or Med Ins carrier.
Tag Line 6; Your blood type & date of birth.
This is suggested information, the most important item is who you are and who to contact if you have a medical emergency/accident.
